Passport Requirements:

Original, signed passport valid for 6 months beyond stay in Ukraine, with at least two blank passport pages available for Ukrainian visa stamp. Amendment pages in the back of the passport are not suitable for Ukraine visas.

Ukrainian Visa Application Form:

One original Ukrainian visa application form properly completed, legibly typed or printed in ink in block letters, signed, plus one copy. Improperly completed Ukraine visa applications may delay the processing of your missionary visa.

Photo Requirements:

One recent, identical, 2 x 2 passport photograph, in color, front view and with a plain/light background.


OR - you can upload a passport photo to the Global Service Order Form when you place your order, instead of providing physical passport photos.

Letter Of Invitation from Ukraine:

An original detailed letter of invitation from the host organization or sponsor in the Ukraine explaining the relationship to the applicant and the purpose of travel to France. In addition, the letter must indicate the length of stay, place of stay, and host’s name, residential status and contact information. Under the signature, the applicant will need the Seal of an official of the Ministry of Culture of the Ukraine on Religious Matters or the department on religious matters of the city where the sponsoring organization is registered.

Special Instructions:

Within 45 days from the moment of entry into the Ukraine, a holder of the long-term one-entry visa (Missionary Visa) should legalize his status in Ukraine (apply to relevant authorities in Ukraine for a permanent/temporary residence permit or special stamp to the passport).

Global Service Order Form:
