A valid passport and missionary visa are required to travel to Ukraine.

Passport Requirements:

Original, signed passport valid for 6 months beyond stay in Ukraine, with at least two blank passport pages available for Ukrainian visa stamp. Amendment pages in the back of the passport are not suitable for Ukraine visa stamps.

Ukraine Visa Application Form:

One signed and notarized Ukraine visa application form. After placing your order, fill out the application form found in your application kit.

Washington, D.C. and San Francisco:The Consulate of the Ukraine has instituted a web-based visa application form. The application must be completed online and then printed. Details can be found in your application kit. We cannot process your visa request if you do not complete the application form online. The printed confirmation page must be included with all other supporting documents needed to obtain your visa.

Photo Requirements:

One recent, identical, 2 x 2 passport photograph, in color, front view and with a plain/light background.


OR - you can upload a photo when you place your order, instead of providing physical passport photos.

Letter Of Invitation:

An original detailed letter of invitation from the host organization or sponsor in the Ukraine explaining the relationship to the applicant and the purpose of travel to Ukraine. In addition, the letter must indicate the length of stay, place of stay, and host’s name, residential status and contact information. Under the signature, the applicant will need the Seal of an official of the Ministry of Culture of the Ukraine on Religious Matters or the department on religious matters of the city where the sponsoring organization is registered.

Proof of Sufficient Funds:

San Francisco: You must provide proof that you possess sufficient funds. Your financial details must:

  • Include 3 of your most recent bank statements
  • Display your full name

Special Instructions:

Within 45 days from the moment of entry into the Ukraine, a holder of the long-term one-entry visa (Missionary Visa) should legalize his status in Ukraine (apply to relevant authorities in Ukraine for a permanent/temporary residence permit or special stamp to the passport).
